I have been thinking about getting a lip augmentation. I have a really cute little Cupid’s bow mouth but that is the problem, it is little.

Question: I have been thinking about getting a lip augmentation. I have a really cute little Cupid’s bow mouth but that is the problem, it is little. I would like to get my lips augmented with injectable fillers but I am worried I will lose that nice shape I have. Can the shape of the lips be precisely controlled during an injectable filler lip augmentation?

Answer: Yes, the shape of the lips can be precisely controlled with lip filler augmentation but I would highly suggest that you seek out a very experienced injector to make sure that you get the result that you desire. It is always my goal when injecting lips to create a very natural-looking lip that is full and luscious but certainly does not draw the eye or have any unusual characteristics that would make someone else think that the lip has been augmented. Some lips require customization so that a Cupid’s bow can be created while other lips require customization so that the natural Cupid’s bow and shape of the lip is retained and yet the fullness of the lip can be improved. I would recommend that you stick with injectable fillers such as Restylane and Juvederm Ultra so that unexpected consequences or complications are avoided.

Question: I am thinking about getting a permanent lip augmentation with an implant to help my modeling career take off. I am 25, and I love the feel of my lips. The one thing I am concerned about is the implant will feel unnatural. Do lip implants feel just like normal lips?

Answer: It depends on what type of implant is placed in the lips. Gore-Tex implants are rather firm and can be felt inside the lip though, typically, if someone is kissing someone with an implant, you would not be able to tell but, if you grasp the lip with your fingers, you would easily be able to feel the implant. Newer soft silicone implants are less obvious and are softer but I would just caution you that, as a model, you may want to consider filler injections due to their extreme safety profile and reversibility with hyaluronidase. There are cases of complications due to lip implants and, though this is rare, could cause scarring in the lip that could affect your modeling career. I would therefore seek out an experienced injector and recommend that you stay with the hyaluronic acid fillers such as Juvederm Ultra and Restylane so that you can get the result that you desire while minimizing any risks.

Question: I am a 38-year-old woman, and I was thinking about getting a mini facelift.  Then I heard that laser peels can make the skin look much more youthful and fresh.  Would a laser peel give comparable results to a mini facelift for about the same price?

Answer: I personally put mini facelifts and laser peels in two completely different categories, and I discuss this with my patients during consultations.  A mini facelift is going to treat early jowling and laxity under the chin, which a laser peel will not treat.  A laser resurfacing, however, will treat wrinkling and discolorations of the skin, which a mini lift will not treat significantly.  Many patients require a combination of both a surgical lifting procedure as well as a skin rejuvenation procedure to give them the appearance that they desire.  It is extremely common for me to perform multiple different procedures at the same time in patients wishing to undergo facial rejuvenation as all areas of the face age simultaneously and, in most patients, the rejuvenation of all areas of the face will give them the most natural and most significant improvement.  I would recommend sitting down with a board-certified facial plastic surgeon so that all of your options can be discussed and a customized treatment plan can be created.

Question: I am in my late 60s and am contemplating eyelid  surgery.  I heard that I will be able to see better after the procedure  is done.  Is this true?

Answer: As far as your visual acuity goes, no, you will not be  able to see better due to a blepharoplasty as it will not change your  ability to focus your eyes.  Some patients with a significant amount of  upper eyelid laxity can have so much skin on their eyelids that it  literally hangs over the edge of the eyelid and can obstruct portions of  their vision, causing what is called a visual field deficit.  For these  patients, they may be able to see more in their visual field after  blepharoplasty but they are not necessarily able to see better.  When  patients do have a distinct visual field deficit due to upper lid excess  skin, insurance can frequently cover the cost of an upper lid  blepharoplasty procedure.  The far majority of my patients, however, do  not meet the specifications to have insurance pay for their upper lid  blepharoplasty and, therefore, the far majority of my patients have this  procedure performed for cosmetic reasons.


I saw amazing before-and-after pictures of a neck lift, and I am interested in learning more about the procedure.

Question: I saw amazing before-and-after pictures of a neck lift, and I am interested in learning more about the procedure.  Would the neck stay lifted or would it begin to sag after a while?

Answer:
A neck lift or what I call a mini lift can do a wonderful job of improving upon a patient’s jowling and laxity under the chin or the area that we would consider the far upper neck.  While this improvement is considered permanent in that a patient will always look better after the procedure than they would have had they never had the procedure done, it is nonetheless going to age and recur in the future.  The longevity of a neck lift is highly dependent on the style of neck lift performed as well as the genetics and age of the patient.  When I perform a neck lift in conjunction with an extended SMAS deep plane facelift, I tell my patients to expect approximately 10 years of improvement before they will look similar to what they did before the procedure was performed.  When I perform the neck lift alone in the form of a mini lift in the office setting, it is highly dependent on the patient’s age as to how long the neck lift will last.  In some patients, they may see a small amount of laxity occur even in the first few months after the procedure, though they still look much, much better than they did prior to having the mini lift in the first place.  Those patients who have early laxity recur are typically in their 60s or older, and we do discuss the fact that the results from the mini lift may only last for three to five years.
