Can injectable fillers be an option to revision rhinoplasty?

Question:
I have very uneven nostrils ever since my nose job a year ago. I know revision rhinoplasty is an option, but could injectable fillers be a good alternative for this?

Answer:
I would have to evaluate your particular anatomy, but it would be very unlikely that injectable fillers would be able to correct uneven nostrils and more than likely revision surgery would be your best alternative.


I have a pinched tip since my nose job. Can this be fixed?

Question: I have a pinched tip since my nose job. Can this be fixed?

Answer: Yes, a pinched tip can definitely be fixed after a rhinoplasty, but it typically requires placement of a variety of cartilage grafts to essentially reconstruct the tip so that you have a more aesthetically pleasing shape to it and better structural integrity since a pinched tip is often caused by overresection of cartilage, which results in a lack of structural support for the nasal tip.  


My nostrils appear very asymmetrical since my rhinoplasty.

Question: My nostrils appear very asymmetrical since my rhinoplasty. What are my options for fixing this?

Answer: If your rhinoplasty surgery was less than a year ago, I would just recommend patient observation since there certainly can be rather significant changes that occur with the nose during the healing phase.  If it has been more than a year since your rhinoplasty then more than likely you will need some sort of revision surgery to correct significant nostril asymmetry.  Fortunately, this can oftentimes be performed through a rather minor revision in the office setting as opposed to needing a full, major revision rhinoplasty procedure.  That obviously is determined by your specific problem and nasal anatomy. 


I’m African American and looking to get a nose job.

Question: I’m African American and looking to get a nose job. I love Rihanna’s nose! Could I have mine made to look the same? Is it possible her nose wouldn’t look good on me even though we’re both African American?

Answer: First of all, it is impossible to make a nose from one person look exactly like the nose of another and, therefore, we would not be able to make your nose look exactly like Rihanna’s but depending on your own specific anatomy, you certainly have the chance of having a nose that looks similar to Rihanna’s.  Ultimately, we have to use the nose that we are given as a starting point for what we are going to create through the use of a rhinoplasty operation.  As such, there is only so much change that we can create and only so much improvement that can be obtained through modification of the existing tissue.  There certainly are patients that have the anatomy that allows us to create exceptional outcomes, but then there are others that have anatomy that make it much more difficult to create a well-defined and aesthetically pleasing nose.  It would really depend on your own personal anatomy as to what can actually be created through a rhinoplasty procedure, but the goal overall is to create a nose that is less noticeable and has better harmony with the remainder of your face.  So, even if we cannot create a nose that looks a lot like Rihanna’s, we should be able to create a nose that goes better with your facial features and does not draw attention to itself, which is the ultimate goal of any rhinoplasty.  This will give you an overall significantly improved aesthetic appearance no matter what your nose looks like today.


Does the degree of swelling after rhinoplasty have more to do with what was done during surgery or with the patient?

Question: Does the degree of swelling after rhinoplasty have more to do with what was done during surgery or with the patient?

Answer: The degree of swelling from a rhinoplasty can certainly be due to the surgeon’s technique in that surgeons that are more refined in their technique and do not damage the surrounding tissue can often end up with less swelling than surgeons that are less experienced or are not as technically gifted.  I do, however, feel that patients with multiple cartilage grafts also tend to have more swelling due to the fact that more trauma is experienced and the fact that the cartilage graft itself may allow for more inflammatory change to occur during the healing process and then the patients themselves certainly can also have an impact on the amount of swelling since obviously some patients will swell more from a traumatic injury than others, so I really feel it is most likely a combination of a variety of factors that will determine how much swelling a rhinoplasty patient gets and how quickly the swelling will subside in those patients.
